Report Improper Conduct
The Susan G. Komen for the Cure ("Komen") Code of Conduct ("Code") requires directors, officers, employees and volunteers to observe high standards of business and personal ethics in the conduct of their duties and responsibilities in pursuit of Komen’s mission. As representatives of the Komen organization, we must practice honesty and integrity in fulfilling our responsibilities and comply with all applicable laws and regulations.
Komen encourages concerned individuals to report any improper conduct by Komen, its Affiliates or their respective directors, officers, employees or volunteers. An individual may report such matters by: i) contacting his or her local Komen Affiliate or Komen Headquarters, (ii) using the reporting submission box below to submit a confidential report or (iii) mailing a confidential report to Susan G. Komen for the Cure, 5600 W. Lovers Lane, Suite 116 #119, Dallas, TX 75209-4330, U.S.A. (this address is an independent mail forwarding service that will delete the claimant's identity and then forward the correspondence directly to Komen’s Legal Department).
Individuals may also report concerns regarding questionable accounting, auditing or other business matters on a confidential basis. Such reports may be submitted by mail addressed as follows: Director - Audit and Risk Management, Susan G. Komen For the Cure, 5600 W. Lovers Lane, Suite 116 #119, Dallas, TX 75209-4330, U.S.A. (this address is an independent mail forwarding service that will delete the claimant's identity and then forward the correspondence directly to the Director- Audit and Risk Management).
All communications will be processed confidentially. Individuals may freely report such information with the assurance that good faith reports will not result in any form of retribution or retaliation by anyone affiliated with the Komen organization.
Anyone reporting a concern may request a response summarizing the results of the investigation of the matter. Persons who elect to identify themselves may request the summary response in the initial report. Requests for a report on the results of the investigation may also be made anonymously through the independent mail forwarding service, which will notify Komen of the request but not disclose the individual's identity or address. This procedure may be invoked by directing the request for a report on the investigation (which should include a reference to (i) the initial report submission date, (ii) nature of the matter and (iii) a confidential address to which the closing report should be mailed) to Susan G. Komen For the Cure c/o Anonymous Whistleblower Reporting, 5600 W. Lovers Lane, Suite 116 #119, Dallas, TX 75209-4330, U.S.A. Komen will submit the closing report to the mail forwarding service, which will forward it to the anonymous whistleblower's designated address on a strictly confidential basis.
